Form 1099-SA (HSA Distributions) and 5498-SA (HSA Contributions) - BenefitWallet

Forms 1099-SA (for HSA distributions) and 5498-SA (for HSA contributions) are both sent out by BenefitWallet.  The forms must be posted to Benefitwallet's website by 1/31, and they also get mailed to your home.  Members can access the form by logging into www.mybenefitwallet.com and selecting ‘My Tax Forms’ from the Member Services tab.
