Form 1099-SA (HSA Distributions) and 5498-SA (HSA Contributions) - Health Savings Account (HSA) Tax Forms

Forms 1099-SA (for HSA distributions) and 5498-SA (for HSA contributions) are sent to Health Savings Plan (HSA) account holders by the HSA benefit administrator.  The forms must be posted to the administrator's website by 1/31, and will be mailed directly to account holders. 

Due to the HSA administrator change from BenefitWallet to HealthEquity in April, 2024, HSA account holders will receive 2024 tax forms from BenefitWallet and/or HealthEquity, depending on the time period during which the HSA was Active.
